SUBJECT: Notification of Injury - Jeremy Steele 

I'm just sending off a quick email to let you know that we have had an accident in the warehouse.  

One of our store hands, Jeremy Steele, was carrying some stock down the steps to the dispatch area when he slipped and fell. He seems to have twisted his arm and hurt his shoulder. I'm not sure if it's dislocated - the first aider thought he should go straight to the doctor. 

It was quite a bad fall. I think it'll be a workers comp claim for sure. 

Just letting you know so you can do whatever you have to do to get the paperwork going.
