Thanks for your message earlier. It's good you let me know how Jeremy is handling things. If he is having those problems with the suitable duties, we will need to change his workload.  

If he's required to use the phone while using the computer, he should be provided with a head-set so he won't need to hold the phone in the crook of his neck.  

And, at this stage, he shouldn't be doing data entry at the computer - or any kind of keyboard work or using the mouse - for longer than about 20 minutes every hour. And he should have a 5-minute break after every 10 minutes he spends keying at the computer. 

I'll change the medical certificate so that it spells this out. Make sure both his supervisors know what's required. You'll need to amend the return to work plan as well.
